2. Initial Nurse Consultation

   A registered nurse from Salusive Health will contact you to discuss your personalized care plan. This may involve sending a kit with health monitoring devices directly to your home. The setup process is straightforward and can be completed within minutes, with support provided throughout.

3. Utilization of the Mobile App

   The mobile application enables you to record your vital signs, track medication adherence, and document any symptoms or inquiries for evaluation by a Salusive Health nurse. This information can be collected and shared from virtually anywhere.

4. Ongoing Health Monitoring

   Your health will be continuously monitored by our nursing staff, who will proactively reach out for follow-ups, address any concerns, and make adjustments to your care plan as necessary. In cases of significant findings, your healthcare provider will be promptly notified. Additionally, providers and insurers can monitor patient progress in real time through a web-based portal to facilitate timely follow-up for those who require it most.
